Student Internship Agreement and Guidelines and Policies: Details the rules and regulations about insurance and personal conduct.
Form 139: Internship Information/Course Description: Contains all primary and academic information about the internship. Note: International students completing internships outside the U.S. should use this form. For internships within the U.S., use Form 110 below.
International Students - Complete Form 110 instead of Form 139 if completing an internship within the U.S. Note: This form must be completed and approved by the Director of International & Off-Campus Programs BEFORE beginning ANY off-campus employment or internship.
Form 200: The Learning Contract: Written by the student in consultation with both the faculty sponsor and site supervisor stating the learning goals and strategies. Due by the end of the first week of the internship or by the final drop date for the semester, whichever is earlier. |

Due at the beginning of the internship:
Memo of Understanding: The site supervisor signs this agreement acknowledging the relationship between the College and the site. Please return this form within one week of the intern's start date.
Due during the internship:
Form 300: Mid-Internship Peformance Evaluation: The site supervisor evaluates the performance of the intern. This form is due at the mid-point of the internship. The Internship Coordinator will contact you with a due date.
Form 400: Internship Final Performance Evaluation: The site supervisor evaluates the performance of the intern. This form is due at the end of the internship or within one week of the end of the semester, whichever is earlier. |
Due during the internship:
Form 301: Mid-Internship Self-Performance Evaluation: The intern evaluates his/her own experience. This form is due at the mid-point of the internship.
Form 401: Intern Final Self-Performance Evaluation: The intern evaluates his/her performance. This form is due at the end of the internship.
Form 500: Student Internship Evaluation: The intern evaluates the Internship Program & experience as a whole. |
